2008 KINDERGARTEN REGISTRATION, CHILD FIND DEVELOPMENTAL SCREENINGS, AND LINCOLN COUNTY HEAD START REGISTRATION

Children who are birth through five years of age are invited to a screening for the developmental areas of speech, language, vision, hearing, motor, dental, nutrition and classroom readiness for kindergarten.

THIS IS KINDERGARTEN REGISTRATION FOR CHILDREN WHO WILL BE FIVE BY SEPTEMBER 1, 2008

Kindergarten Registration:
Birth Certificate and Immunization Record
All other children:
Immunization Record

Public Health will update immunizations for those who bring their records.
Medicaid on Site Application Assistance (MOSAA) Available
